Free COVID-19 Digital Screening Tool

*Disclaimer: This tool is meant for healthcare professionals/organizations to use for their patients. This tool is currently unavailable for individual use by the general public.

As a team made up of mostly physicians, we believe it is our duty to put patients before business. That's why to help do our part, we're providing clinics our intelligent intake platform completely free of charge to primary care providers, clinics, and health systems during the Coronavirus Disease 2019 (COVID-19) pandemic.

Our goal in providing this content via our remote patient intake platform is to help reduce community exposureaugment telehealth visits, compliment phone triage working at full capacity and provide physicians clinically relevant information for patient care. Here is how our platform can specifically help:

1.CDC Guideline-based COVID-19 Risk Screenings

Per CDC provider guidelines, our intake system screens patients for recent travel to geographically-affected areas or exposure to laboratory confirmed COVID-19 cases who have any of the following symptoms:

  • Cough

  • Fever

  • Difficulty breathing/shortness of breath

  • Fatigue

  • Headache

  • Muscle soreness

2. Domestic Cluster Exposure

Disclaimer: This screening has not been approved by the Centers for Disease Control and Prevention (CDC), Food and Drug Administration (FDA), or for publication in peer reviewed journals. The content's sole purpose is to collect and provide supplemental information to clinicians. This content is not meant to diagnose.

With the help of tools like the active case map provided by the New York Times, we've taken the initiative to screen patients for recent domestic travel to known clusters and outbreaks within the United States. This screening includes the following:

  • Cruises

    • Personal travel on Grand Princess cruises

    • Exposure to people who traveled on Grand Princess cruises

  • Washington state

    • Personal travel to nursing facilities in Kirkland or Standwood

    • Exposure to anyone who traveled to nursing facilities in Kirkland or Standwood

  • Massachusetts

    • Personal attendance at BioGen conference in Boston, MA

    • Exposure to anyone who attended the BioGen conference in Boston, MA

  • New York

    • Personal travel to New Rochelle, NY

    • Exposure to persons who traveled to New Rochelle, NY

3. Mortality-associated Comorbidities

Our physician team reviewed the currently available literature to determine associated medical conditions in non-survivors compared to survivors of COVID-19. Using these recent data, we've developed content to screen for the following demographic and clinical characteristics with an odds ratio (OR) greater than 2:

  • Current smoking (OR=2.23, p=0.20)

  • Chronic obstructive pulmonary disease (OR=5.40, p=0.056)

  • Coronary artery disease (OR=21.40, p=<0.0001)

  • Diabetes (OR=2.85, p=0.0062)

  • Hypertension (OR=3.05, p=0.001)

4. (In Progress) Risk Stratification

Our team is rolling in a stratification system to help healthcare providers triage patients based on patient risks of exposure and outcomes. These metrics were  developed in consultation with primary care physicians, the CDC guidelines, and review of COVID-19 clinical studies.

Please note the mortality risk metrics have been hidden to minimize anxiety among non-healthcare professionals. Exact metrics behind the stratification will be provided to healthcare providers.

Exposure Risk

Low

No CDC guidelines met.

No exposure to domestic outbreaks.

High

1+ CDC guideline met

OR

Exposure to domestic outbreak

Morbidity Risk

Low

Young age

No medical conditions

Moderate

Middle age
OR

1 comorbidity

High

Elderly
OR

2+ comorbidities

5. Mental Health Screenings

During any natural disasters, patients are under greater levels of stress. As a result, we anticipate increases in depression and anxiety disorders due to the acute stress of the pandemic. In addition to respiratory symptom investigation, we'll be screening all patients for major depression, adjustment disorder, and other mood or anxiety disorders by delivering PHQ-9 and other mental health assessments.

Remote Patient Intake

Our platform can connect to your EHR and telehealth services so patients can complete the screening remotely on their phone or computer at home.

Rapid, Fully-remote Onboarding

We're shifting a majority of our staff and resources to address the immediacy of this crisis by providing the following features at no-cost:

  • <48 hours/clinic onboarding

  • Remote orientation

  • 24-hour live tech & product support

  • Direct line to leadership

  • On-demand, as-needed customizations
